SWAN and County/Agency Services
Adoption Connection, PA is an Affiliate of the StateWide Adoption Network (SWAN), because strong inter-agency relationships are key to successful family permanency. We provide a variety of professional services to county and other agencies; our representatives also advocate for families within these various agencies to be sure that everyone involved is well informed and prepared throughout the adoption journey.

Listed below are the agency services and products provided by Adoption Connection, PA:
Home Study
A Home Study is a report that consists of various paperwork, references, background checks and physician's exams, as well as visits to your home for interviews and a home inspection. This process allows us to compile a candid portrayal of your home and family in a document that can be used nationally. International Home Studies are also available.
Family Profile
This extensive process is used by a licensed adoption agency to determine the suitability of a prospective individual or family’s qualifications to parent a specific child, based on that child’s specific needs. It is similar to the Home Study, but can only be used within the Commonwealth of Pennsylvania.
Foster Care
The primary goal of our foster care service is to provide opportunity for children who enter the child welfare system to go directly to the home of a certified foster care family who is willing to support (A) the child’s placement in a pre-adoptive family or (B) the child's possible reunification with the child’s birth family.
Child Profile
A Child Profile is a review and assessment of the child’s life to assist in identifying a permanent family, to identify strengths and risk factors, and to maintain a history for the child.
Child Preparation
Child preparation involves the development and implementation of a written plan for the planning and preparation of the child for a permanent placement.
Child Placement
Child placement involves the implementation of the plan for placement and family selection by the county agency.
Child Specific Recruitment
This is a services tailored to an individual child's permanency goals.
Adoption Finalization
This service includes all services provided to the child, the adopting family and the county agency with custody of the child leading to the issuance of a decree of adoption by the court.
Crisis Intervention
Through direct Case Management this service may include on-call home visits, ongoing counseling and/or other supportive services to directly intervene during emergencies or to reduce the probability and/or prevent placement disruption.
Post Permanency Services
Through Case Management, this service helps families obtain outside assistance to help reduce the probability and/or prevent adoption placement disruption.  Respite Services, Support Group and other services extend the family's support system and offer reprieve for the family while providing a safe and supportive environment for the child.
