Connecting Home is Adoption Connection, PA’s After Care Preservation Program.
The Connecting Home program currently serves Allegheny and Washington Counties.
Objective:
Our reunification/permanency preservation program builds on principles of shared parenting and family time coaching and aims to support families in maintaining stability and promoting positive outcomes following reunification or placement into permanent homes. Through personalized coaching, emotional support, and community resources, we strive to enhance parenting skills, strengthen family bonds, and reduce the risk of re-entry into foster care. Our vision is to empower families to thrive and provide a nurturing environment for their children.
The Program Focuses on:
· Rebuilding Family Relationships: Helping caregivers and children rebuild and strengthen their relationships after being separated by the foster care system is the primary goal. This involves promoting healthy communication, trust-building, and fostering positive family dynamics.
· Parenting Skills Development: Supporting caregivers in developing effective parenting skills is crucial for keeping a stable and nurturing home environment. This may include further education on child development, positive discipline techniques, and strategies for managing stress and challenging behaviors.
· Strengthening Family Functioning: Enhancing overall family functioning by addressing areas such as conflict resolution, decision-making, and problem-solving skills. Building resilience within the family unit helps them navigate future challenges more effectively.
· Preventing Future Child Welfare Involvement: Ensuring caregivers remain safety focused involves equipping families with the tools, resources, and support networks needed to address challenges independently and maintain a safe and nurturing environment for their children.
· Creating Community Connections: Connecting Home helps to build connections with community resources, support networks, and other families who have experienced similar challenges. Building a dedicated support system within the community provides families with added sources of help and encouragement.
