Permanency Caseworker (SWAN):
Adoption Connection, PA is looking for an outstanding individual to add to our rapidly growing team! The Permanency Caseworker implements all of the SWAN units of service including but not limited to Child Profiles, Child Preparation Services, Child Specific Recruitment, Family Profiles, Placement and Finalization for families in our community. We provide services in the home and community settings.
The Permanency Caseworker will work closely with referral sources in order to develop a plan in collaboration with the client and the family. This position requires scheduled home visits for all services provided, scheduling and attending record reviews with outside agencies, writing reports following visits in accordance with Statewide Adoption and Permanency Network (SWAN) protocol, attending court hearings when necessary, and ensure all deadlines are met.
A Bachelor’s Degree is preferred but experience will be accepted in place of degree. The office is located in Beaver, PA and you will be required to be in the office at least 4 days per week as well.

Contracted and Full-time Transportation Case Aids (Beaver and Washington Counties):
- Case Aide provides transportation and/or supervision (day, evenings & weekends) between youth in foster care and their birth parents as assigned, coordinate visitation as assigned, and document all transportation and visitation. Work locations can vary due to the needs of the children we serve and can be spread around many different counties in the area.
- A Case Aide must demonstrate strong mentoring skills for the individuals that they will be working with.
- This position requires: High School Diploma (or GED), a flexible schedule with evening availability, access to a computer, ability to obtain Act 33/34 clearances as well as FBI clearance. Must be responsible and dependable with a clean driving record, valid drivers license, and reliable transportation.
